Our kula will once again host Bay Clinic for a vaccination clinic this Friday, February 11from 9 a.m. to 12 p.m. at Lunalilo Hale. The clinic is open to KS students and employees ages 12+ who need to receive a booster (or are interested in initiating or completing a vaccination series), with the added ease of being on-campus.
Last month, the Hawaiʻi Department of Health revised and clarified its definition for individuals to be up-to-date on their vaccinations now means a completed series or booster shot received in the past five months. Only those who are up-to-date are exempt from both a five day quarantine and restrictions over participation in high risk activities.
Everyone 12 years and up are now eligible to receive a booster, if it has been at least five months from your last dose (or 2 months if 18+ and you have received Johnson & Johnson).
The vaccination process can take 30-45 minutes from intake, receiving the vaccine, and a mandatory observation period.
ʻOhana should complete the sign-up form so their student can be scheduled into an appointment time on that day. Please sign-up by Wednesday, February 9 at 4 p.m. so schedules can be made for when students are released. The high school or middle school offices will let students know when they are released to go to the clinic. Employees may walk-in at any time in the 9 a.m. - 11:45 a.m. window.

If you have received a vaccination from Bay Clinic in the past, you do not need to complete the registration packet. Just complete the last two Pfizer forms (screening and administration record).
PLEASE HAVE YOUR KEIKI BRING THESE COMPLETED FORMS WITH THEM TO THE CLINIC. If students do not have completed forms, Bay Clinic will not be able to administer their booster or vaccination shot at that time.
For anyone 18 & older - forms for Moderna and J&J will be available on-site if choosing those options.
If you are receiving your booster or a 2nd dose, please remember to also bring your vaccination card.
Important note:
While there is no out-of-pocket cost to receive the vaccination, there may be an administration fee billed to an insurance carrier, third-party payer, Medicare or Medicaid for this service. Participants are asked to provide a copy of their Health Insurance card and i.d., so that Bay Clinic may process this claim.
