KSH is excited to put out the 2025-2026 School Year Athletics Interest Survey!

High School students interested in joining one of our athletic teams next year?

Fill out the KSH Athletics interest survey and compete for our Warriors next school year! I Mua Kamehameha! 

Please note: the following requirements are needed for eligibility for tryouts/participation.

• Physical Exam Form Valid for 13 months from physical exam date (i.e. physical exam date of 02/16/2022, the physical expires on 03/16/2023 for athletic participation). Upload updated PE form to the Mo’omōʻali Olakino (EHR) portal at https://ohana.ksbe.edu/. PLEASE ENSURE THAT FORMS ARE UPLOADED TO THE STUDENTS PROFILE NOT THE PARENTS PROFILE. 
   
• Concussion Baseline Current/incoming 9th and 11th graders are required to complete an online concussion baseline assessment. If needed, student athletes will receive an email at their school email from “AtHomeTest@ impacttest.com” with a link to take a baseline concussion test.

PARENTS: Please ensure that you and your student are not BOTH filling out a form. 

STUDENTS: Please do not fill out form for other students.
