Aloha e Parents,

Kula Waena Administration will be using KS Connect to update your child's Student Directory of Information (SDI) forms for the 2020-2021 school year. You will be able to update your child's SDI forms online by logging into your KS Connect Parent Account starting April 1, 2020. Online SDI forms will close on May 15, 2020. Please be sure to update your information and complete ALLschool forms needed to cleared your child for the 2020-2021 school year. We have provided a link with helpful instructions on completing this process.

Required Forms:

• SDI (Student Directory of Information) (Only 1 Parent/Guardian needs to complete this form)
• Tuition / Financial Clearance    (Parent/Guardian(s) sign)
• Enrollment Agreement    (Parent/Guardian(s) & Student sign)
• General Release, Waiver of Liability and Indemnity Agreement    (Parent/Guardian(s) & Student sign)

Please be sure to check your EHR parent portal for your child's medical clearance status.
