Please complete the "Student Daily Wellness Online form" each day for any Kamehameha Schools students coming to campus for school, please complete it by 7:00am or before student gets dropped off or arrives on campus. If you answer all of the questions and receive a green happy face, you can come to school, if you receive a red sad face, that means you are not able to come to school.

For students being dropped off at the curbside, you must show your green happy face to the KS staff person on morning duty at curbside before the student can leave the vehicle.
For students riding the bus, a report will be pulled each morning, so when the students arrive on campus, the KS staff person will check to see if the student bus rider is cleared and on the list. IF your student is not cleared and on the list, they will need to go to the school office and the parents will be called. If the form is not filled out by 8:15am when school starts, the parent will be asked to come to campus and pick up their child.
When filling out the form each school day that your child is coming to campus for school, on question #2, please be sure to input your childs KS email (First Name.LastName@imua.ksbe.edu) and please be sure to spell it correctly. Read the form carefully, some of the questions have been revised, fill out all of the mandatory areas and then hit submit.
IF you have multiple children attending Kamehameha Schools, please fill out a daily wellness form for EACH of them, they cannot be on one form.

REMINDER: Mornng Drop Off and Afternoon Pick Up - Students whose LAST NAME begin with A to K, please drop off and pick up at the Paiʻea Courtyard curbside. Students whose LAST NAME begin with L to Z, please drop off and pick up at the Nāmāhana Circle. Please put your students name placard on your vehicles dashboard and please be ready to present your student's daily wellness clearance, the greenhappy face.

IF your student is going to be out from school, here is what you need to know to allow for your student to return to school.
If your child is experiencing any symptom(s) of COVID-19, for any length of time including only ONE day, even if at home and not missing any learning, a doctor's note or negative COVID -19 PCR test is required! These documents must be provided to your child's health room PRIOR to your child's return to school or school activities. The previous return to school policy applies for any other reason for missing school. IF your child is absent due to a medical reason, but not experiencing COVID symptoms or any other COVID related reason, a parent note or PCP note will be required if absent for 1-3 days. A PCP's note continues to be required for return following 4 or more days of illness. Please see the other message in the daily bulletin for more details.
We offer COVID - 19 testing here at school. Saliva based PCR testing is available, voluntary and at no cost to your student. Testing is intended to help improve the safety of our campus community. Testing will occur on campus on schools days, Tuesday thru Thursday. A consent form must be signed in the KS electronic health record Moʻomōʻali Olakino.
Parents/guardians must provide required documentation as determined by criteria below. Students can take parent notes to the administrative office upon their return. If other documentation, e.g. COVID-19 test result or healthcare provider note, is required, it should be uploaded to the EHR (Electronic Health Record) through http://ohana.ksbe.edu/ > Moʻomōali Olakino BEFORE the student returns to school. If it meets criteria, Health Services staff will notify the parent/guardian that the student is cleared to go directly to the administrative office, after any campus symptom screening check/thermal scanning.
If the student is not pre-cleared to return to school, as described above, the student must report directly to the health room with the required documentation BEFORE reporting to the administrative office.
Requirements for Returning to School Following Absence due to Illness or Injury:
1. Student has been fever-free for at least 24 hours without the aid of fever reducing medication such as Tylenol/Ibuprofen.
2. Students symptoms have resolved and can answer ‘’no’’ to the student symptom screen questionnaire.
3a. Absent 1-3 days WITHOUT COVID like symptoms: Parent note is required.
3b. Absent 1-3 days WITH COVID like symptoms: Healthcare provider note that clearly states the student is cleared to return to school OR a negative COVID PCR test collected after the onset of any symptoms is required.
3c. Absent 4 or more days: Healthcare provider note that clearly states the student is cleared to return to school is required. If the student tested positive for COVID or was deemed a close contact of someone with COVID, documentation from a healthcare provider or the Department of Health releasing the student from isolation (if positive) or quarantine (if a close contact) is required.
The Centers for Disease Control and Prevention (CDC) identifies the symptoms most commonly associated with COVID-19. They are generally the same for adults and children and are:
*Fever or Chills
*Shortness of breath or difficulty breathing
*Muscle or body aches
*New loss of taste or smell
*Congestion or Runny Nose
*Nausea or vomiting
These are the symptoms that are asked about in the KS Daily Wellness online form, which now asks if your child has had any of these in the previous ten days. They are also the symptoms for which a student would be sent home from school, if the symptoms are not typical for the student who has a known chronic condition that causes them.
