Instructions for completing the Hawaiian Ancestry Registry form

A. Applicant Information
1. Legal Name – Enter complete legal name of applicant (no nicknames). Be sure to use the same name for any program applications. Also list other names, previous married names, and complete name at birth if different from current legal name.
2. Adopted? – Do not leave blank. Select yes for all adoptions, including adoptions by a step-parent. (While hänai relationships are NOT considered adoptions, they also cannot be used to trace ancestry.)
3. Gender, SSN, DOB – All fields are required. Because there are many common family names in Hawai‘i, social security numbers and birth dates are required to distinguish between applicants with similar names. This information is used by KS only and is kept strictly confidential.
4. Place of Birth – Enter the city and island of birth if born in Hawai‘i, or state or country of birth if born outside Hawai‘i.
5. KS Graduate or HHDC Registered Sibling – This information is used to assist us in the verification process only. List siblings whose KS files may be a source of additional documents. Include siblings who are current or past KS students (include year of graduation and campus); have previously registered with the Data Center (include Data Center ID number if available); or are concurrently registering with the Data Center.
6. Mailing Address – Use the address of the applicant or person responsible for a minor applicant. All correspondence will be forwarded to this address unless otherwise requested.

B. Signature
The HAR form must be signed by the applicant if he or she is 18 years of age or older, or a parent or legal guardian if the applicant is under 18 years of age. If anyone other than the applicant or birth parent signs the HAR form, a copy of the court order regarding legal authority is required. (This applies to parents of non-minor applicants, grandparents, and non-adoptive step-parents as well.) After signing, PRINT name, relationship to applicant, and a daytime phone number of the person who signed the HAR form.

C. KS Program Information*
Enter the name of any KS program(s) if applying within the next six months from the date the HAR was submitted.

D & E. Ancestry Information – Father’s and Mother’s Family
Enter ancestry information for the BIRTH father and his family, and the BIRTH mother and her family. Do not list adoptive parents or hänai parents. ALL FIELDS MUST BE COMPLETED, even for family members who do not have Hawaiian ancestry. Write “unknown” if the information cannot be obtained. See A. Applicant Information for more details.

Required Supporting Documents

• All birth certificates and other documents must be originals (photocopies will not be accepted) and must be issued and certified by a government or state agency.
• All documents will be copied and returned to you.

Submit original birth certificates for the applicant, at least one Hawaiian parent, and a Hawaiian grandparent (all from one side of the family) to show that he or she is a direct descendent of a Hawaiian ancestor born on or before December 31, 1959. That ancestor must have been born in Hawai‘i and show Hawaiian ancestry on his or her birth certificate. If not, birth certificates must be provided for earlier ancestors until these requirements are met. If both parents are of Hawaiian ancestry, only the ancestry of one side of the family must be verified.

In situations where certified birth certificates do not exist or cannot be obtained due to hardship, alternative documents may be accepted to establish Hawaiian ancestry. Examples of alternative documents are listed below in the preferred order of acceptability:
1. Certificates of Delayed Birth
2. Certificates of Hawaiian Birth, including all testimonies and notations
3. Marriage certificates
4. Death certificates

For adoptions, the following additional documents are required:
• Pre-adoption birth certificate identifying natural parents along with post-adoption birth certificate; OR
• Letter of Non-Identifying Information of Racial Extraction of Biological Parents issued by and obtained from the Family Court in which the adoption occurred; OR
• Adoption papers if the adoption is by a step-parent, or if the ancestry of the natural parents is identified thereon.

How to Obtain Documents
If original Birth Certificates are not available, it is recommended that the applicant order duplicates immediately from the Hawai'i State Department of Health Office of Health Status Monitoring, Vital Records Section at (808) 586-4533 (for Hawai'i births only). More information can be obtained on their web site at http://www.state.hi.us/health or at their offices at 1250 Punchbowl Street Room 130. Please make your request as early as possible because it usually takes four to six weeks to process each request. For births that occurred outside the State of Hawai'i, but within the United States, certified copies of Birth Certificates can be requested through that respective state's Department of Health. For births that occurred outside the United States, certified copies of Birth Certificates can be requested through the American Consulate of the country in which the child was born.*The Hawaiian Ancestry Registry is to verify Hawaiian ancestry only and does not serve as a substitute for an application to any Kamehameha Schools' program, nor does it guarantee admission to any particular Kamehameha Schools' program.
